**PLEASE NOTE** CIGNA Healthcare has sent notices to verify the eligibility of your dependent children ages 19-25 and 25-30.  This is separate from the documentation Martin County School District requested. Please be sure to return the questionnaire to CIGNA by their deadline.  CIGNA will not receive the documentation you provided the District. If the CIGNA forms are not received by their deadline, your dependent(s) coverage will be terminated effective December 31, 2009.
