REMINDER - BENEFIT CHANGES EFFECTIVE JULY 1, 2010
Medical & Dental has changed to Aetna
Employee Assistance Program (EAP) changed to Horizon Health


New ID Cards were mailed the week of June 21st. You can also login to www.aetnanavigator.com and register. Here you can print temporary ID cards, order lost cards, review claims, find doctors, dentists, labs, hospitals, other medical facilities, and much more. If you need additional assistance, please contact the onsite Aetna representative at 772-219-1200 extension 30313 or email wilsond9@aetna.com.

EMPLOYEE BENEFITS CENTER

At the EBC, you can view your current benefit elections, learn about qualifying life events and change your elections accordingly, access many District forms, find insurance carrier phone numbers and websites, claim forms and much more.
